Diagnoses, findings and prescriptions are specially protected data under Art. 9 GDPR — a different legal class from a haircut: explicit consent instead of legitimate interest, tighter purpose limitation, different retention.
That is why every trade carries a compliance profile and individual fields are marked as specially protected. Export, anonymisation and consent checks read that marking. Adding this only once the first practice signs means adding it to an architecture never designed for it.
